You can wear them under your shorts, or under your pants and some people just wear them alone. Its really up to you, but they work great under your clothing as well.

Waist runs true to size.

I have 4 pair of these. They are consistent in manufacturing and waist fits fine, and the pouch is snug, but not too tight. Snug is what you want in a well fitting jock.
